ABSTRACT

@#AIM: To investigate the clinical characteristics and treatment of uveitis associated with ankylosing spondylitis(AS). <p>METHODS: Totally 40 uveitis associated with AS patients(46 eyes)were collected from August 2012 to August 2015 in our department. Data of these patients concerning clinical characteristics, laboratory examinations and treatment were retrospectively analyzed. <p>RESULTS: Of the 40 patients, 28 were males(32 eyes)while the remaining 12 were females(14 eyes). The mean age of the patients at onset of AS was 29.68±7.83 years old, while the average duration was 6.95±6.12a. The history of uveitis of these AS patients was 3d to 6a, with a mean duration of 28±15d. All patients showed typical sacroiliitis with X-ray or CT examination; 37 patients(93%)were HLA-B27 positive; Erythrocyte sedimentation rate(ESR)of 27 patients(68%)increased while 18 patients(45%)showed increased C-reactive protein(CRP). Uveitis of all patients was either cured or under good control after treatment with corticosteroids and cycloplegic eye drops. <p>CONCLUSION: Young males suffer more than female with uveitis associated with AS, which is often characterized by unilateral onset and recurrent relapse. Clinical doctors should pay more concerns on the uveitis patients the onset of AS. Early and enough treatment were needed for these patients.

ABSTRACT

?AIM:To evaluate the incidence and clinical properties of dry eye after phacoemulsification in age-related cataract patients.?METHODS: Samples were collected from 145 age -related cataract patients (145 eyes).Dry eye was analyzed at 0, 7, 30, 90 and 180d after phacoemulsification by 1 ) Ocular Surface Disease Index questionnaire ( OSDI ) , 2 ) tear meniscus height ( TMH ) , 3 ) corneal fluorescein staining, 4) tear film break-up time (BUT), 5)SchirmerⅠtest( SⅠt) .?RESULTS:The symptoms and signs of dry eye, such as narrowing of TMH, shorting of BUT, decreasing of SⅠt, cornea staining by fluorescein, occurred as early as 7d post-phacoemulsification and were measured by OSDI questionnaire and 4 additional clinical tests.Over the six-month observation the severity of dry eye peaked at 30d and then gradually relieved.? CONCLUSION: The severity of dry eye after phacoemulsification peaked at 30d and gradually improved over time. Considering the characteristics of ocular surface for aged people ophthalmologists should pay more concern on evaluating the occurring of dry eye after phacoemulsification so as to improve the life quality of these people.

ABSTRACT

ObjectiveTo investigate the success rate and energy requirements of ibutilide and direct current(DC)cardioversion,summarize the nursing experience.MethodsForty-six patients with persistent atrial fibrillation were enrolled and underwent DC cardioversion were selelct from May 2009 to January 2011.23 patients received pretreatment with ibutilide before DC (the ibutilide group),23 patients received non drugs before DC (the control group).The cardioversion energy started at 100 J and increased if patients could not acquire sinus rhythm.All the patients were given pertinent care before,during and posttreatment.The success rate and energy requirements of the two groups were compared.Results95.7% of the patients were successfully cardioverted in the ibutilide group,which was significantly higher than 74.0% of the control group.The energy required for cardioversion was ( 139.1±45.1 ) J in the ibutilide group,significantly lower than (197.8±53.3) J in the control group.Non patients terminated treatment or appeared complication because of improper nursing.ConclusionsPretreatment with ibutilide enhanced the efficacy of DC cardioversion for the patients with persistent AF at lower energy requirements.Proper nursing care can ensure the smooth process of treatment and decrease the complications.

ABSTRACT

Background The underlying mechanism of choroidal neovascularization(CNV) is multifactorial and complex.Focal adhesion kinase(FAK) plays a crucial role in controlling essential cellular processes and influencing distinct steps of the angiogenic response.But to our knowledge,seldom study on the effect of FAK on CNV formation has been reported previously.Objective In this study,the effect of several CNV risk factors on the expression of FAK in cultured retinal pigment epithelium(RPE) cells was investigated to illuminate effect of FAK on CNV.Methods Human RPE cells were isolated from donor eyes and exposed to H2O2,swallow of outer segment of photoreceptors(POS) and extracellular matrix(ECM) separately with the treating as follows:RPE cells were co-cultured with 10,20,50 and 100μmol/L H2O2 for 20 days;POS(1×106/ml) were co-cultivated with RPE cells for 20 days(setting control group,POS group,hypoxia group with 200μmol/L CoCl2,and POS+hyoxia group);RPE cells were cultured on the plates coated with 100mg/L fibronectin(FN),laminin(LN) or collagen typeⅠfor 30minutes or 1 hour.The expression of FAK and pFAK in RPE cells were examined by Western blot analysis.Results FAK was highly expressed in the 20μmol/L and 50μmol/L H2O2 groups compared with control group(P<0.01);while he expression level of pFAK was reduced after treated with H2O2 in comparison with the control group(P<0.01).After cultured with POS for 20 days,the undigested lysosome could be observed in RPE cells.The expressions of FAK and pFAK in RPE cells were not significantly changed between control group and POS groups(P>0.05),but those in hypoxia group were significantly up-regulated in comparison with control group(P<0.01).Compared with the hypoxia group,the expression amount of pFAK was elevated in POS+hyoxia group(P<0.01).In comparison with control group,the increased pFAK expression was seen in FN,LN and collagen typeⅠtreating for 1-hour groups(P<0.05,P<0.01).Conclusion FAK pathway participates in several CNV-initiated signaling,such as H2O2,POS and ECM,in cultured RPE cells.It is reasonable to believe that FAK potentially plays an important role in CNV-dependent disorder.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the value of the detection of a 4-marker (ER, VIM, CEA and p16) panel in the differential diagnosis of primary endocervical and endometrial adenocarcinomas.</p><p><b>METHODS</b>Immunohistochemical EnVison method was used to detect the expressions of ER, VIM, CEA and p16 in paraffin-embedded tissues from 31 cases of primary endocervical adenocarcinomas and 30 cases of endometrial adenocarcinomas. The specificity, sensitivity, predictive value and accuracy were compared between the 4-marker and 3-marker (ER, VIM and CEA) panels.</p><p><b>RESULTS</b>The positivity rates of ER, VIM, CEA and p16 in endocervical adenocarcinomas were 35.5%, 19.4%, 77.4% and 67.7%, respectively; those in endometrial adenocarcinomas were 70%, 73.3%, 40% and 13.3%, respectively, showing significant frequency differences (P<0.05) between primary endocervical and endometrial adenocarcinomas. The specificity, sensitivity, positive predictive value and accuracy of the 4-marker panel in endocervical adenocarcinomas were significantly higher than those of the 3-marker panel (96.3% vs 90.2%, 65.1% vs 57.6%, 94.9% vs 89.4%, and 85.8% vs 80.6%, respectively). These values were almost similar for both panels in endometrial carcinoma except for better negative predictive value and accuracy value with the 4-marker panel (58.7% vs 51.9% and 75.4% vs 68.6%, respectively).</p><p><b>CONCLUSION</b>Adding the p16 marker to the traditional 3-marker panel may have significant clinical importance in the differential diagnosis of primary endocervical and endometrial adenocarcinomas to improve the diagnostic accuracy, although there is only a slight increase in the diagnostic sensitivity.</p>
